AWARDS NOMINATION FORM

The Awards Selection Committee is now accepting nominations for outstanding individuals in the fields of addiction. Winners will be honored at the 2023 Utah Fall Conference on Substance Use Disorders Conference.  You are welcomed and encouraged to submit your nomination(s) directly to esi@esimgt.com or 801-816-9446 (fax)

BACKGROUND

These awards give individuals or groups in or connected to the Utah behavioral health systems (mental health and addictions) an opportunity to honor those who have made exemplary contributions which have improved understanding and/or enhanced awareness or treatment of substance use disorders.  None of the awards are given out each year as a routine occurrence but rather in the years wherein individuals are recognized for high achievement.

To be considered for the 2023 awards, an individual, program, organization or coalition must have set an example for his or her colleagues and/or made significant contributions in the provision of services in the areas of prevention, recovery and wellness, or treatment.

Individuals in any state, local authority program, private organization, profit or not-for-profit entity may submit nominations. The Awards Selection Committee will select the winners, and recipients will be notified in August. Awards will be presented at a conference luncheon.

AWARDS, DESCRIPTIONS & QUALIFYING CRITERIA

LEON POVEY LIFETIME ACHIEVEMENT AWARD

To be considered for this award, the nominee:

  • Must have made a significant contribution to the field of substance use prevention, recovery and wellness, or treatment over the course of his/her career. (Preference is for nominees with at least 25 years of service in the field.)
  • Must exemplify an ongoing commitment to the field.
  • Must exemplify the highest ethical standards.
  • Must be seen as a leader, through position and/or personal example.
  • Must have made significant contribution(s) to the public substance use system and its clients.

MERLIN F. GOODE PREVENTION AWARD

This special award is to recognize professionals in the prevention field who demonstrate outstanding dedication and commitment to preventing substance use disorders.

Nominees will be judged on the following criteria:

  • Success and contribution in local and state level prevention;
  • Successful collaborative efforts and leadership in their area;
  • Successful collaborative efforts and leadership throughout the state prevention system; and,
  • Success in prevention policy, planning, and practice.

PATRICK J. FLEMING AWARD

The purpose and criteria for this recognition is to honor individuals who have:

  • Increased public awareness and understanding of substance use disorders;
  • Improved the lives of many by advocating for needed prevention, treatment, and recovery support services/programs;
  • Developed successful collaborations with key community stakeholders; and,
  • Demonstrated success in substance use disorder policy, planning, practice, and advocacy.

COALITION AWARD

Coalitions that produce beneficial outcomes in the communities in which they work will be recognized by this award. Nominated coalitions must follow best practices in the field of substance abuse prevention.

Coalitions receiving this award must:

  • Have been in existence for a minimum of 5 years;
  • Be committed to evidence-based prevention;
  • Utilize Communities That Care (CTC) or follow the Strategic Prevention Framework (SPF) to organize and mobilize its efforts;
  • Be engaged in local, state, and national policy including environmental strategies;
  • Demonstrate significant outcomes in reducing risk factors and/or increasing protective factors; and,
  • Demonstrate success in substance use disorder prevention goals.

JUDGE KATHLEEN M. NELSON JUSTICE AWARD

This award recognizes individuals who, through their work or involvement with the legal system, have made significant contributions to:

  • Improve the quality of life of those struggling with substance use disorders and
  • Enhance the health of our communities.

TREATMENT AWARD

The purpose of this award is to recognize individuals who have demonstrated:

  • Excellent clinical skills;
  • A strong personal commitment to clients and peers;
  • Innovative and collaborative leadership in advancing the field of substance use disorder treatment.
